Hashimoto's, fatigue & anxiety:
MSQ 38 to 4 in ten months

A woman in her mid-forties with Hashimoto's thyroiditis, profound fatigue, anxiety and lifelong constipation — achieving a 90% reduction in symptom burden and 75% reduction in TPO antibodies through a comprehensive root-cause programme.

"After years of persistent fatigue and low mood, I had seen other practitioners without fully understanding what was driving my symptoms. Working with Paul was different from the outset. By addressing underlying deficiencies alongside emotional patterns, the shift was gradual but real. Over ten months, my energy returned, the anxiety reduced significantly, and my motivation and confidence rebuilt steadily. I feel like myself again — clearer, more resilient and far more engaged with life."

Presentation & Background

C.B. had been living with a debilitating cluster of symptoms for many years. Thyroid irregularities had been identified in her early thirties — but conventional medication had provided little meaningful improvement in energy. A significant history of chronic stress had placed a sustained load on the HPA axis. She had also experienced chronic candida overgrowth and ongoing immune hypersensitivity — markers of a gut-immune system under long-term strain.

Presenting Symptoms

  • Profound, unrelenting fatigue regardless of rest
  • Apathy — loss of motivation and engagement with daily life
  • Anxiety including difficulty processing at work and social hyperreactivity
  • Low self-confidence and self-esteem, long-standing
  • Constipation from infancy — lifelong sluggish gut motility
  • Eczema on stomach and underarms, worsening with menstrual cycle
  • Chemical sensitivity — strong reactions to perfumes and fumes
  • Perimenopause-related deterioration in energy and resilience

Key Laboratory Findings

  • TPO Antibodies: 291 IU/mL — more than 30 times above reference range (≤9). The defining finding of Hashimoto's. High antibody levels correlate directly with fatigue, cognitive fog, anxiety, mood disruption, and skin reactivity — every one of C.B.'s core complaints.
  • TSH: 32.81 mU/L — severely elevated. The brain was sending urgent distress signals to a thyroid under autoimmune attack that could not adequately respond. This explained the profound fatigue directly.
  • Free T4: 8.8 pmol/L (below reference) — insufficient thyroid hormone output despite medication
  • Ferritin: 28.1 ng/mL (below reference of 30–150) — directly contributing to fatigue and suppressing thyroid hormone conversion. Iron is required for the enzyme that converts T4 to active T3.
  • DUTCH Hormone Test: Low metabolised cortisol — adrenal fatigue at a measurable biological level. This was not a lifestyle observation but a clinical finding that explained the exhaustion and apathy that no amount of thyroid hormone alone could resolve.
  • Dairy Zoomer: 2.8 (above reference ≤2.0) — confirmed dairy as an active immune trigger, explaining eczema flares and sinus symptoms.

The Programme

Autoimmune Protocol (AIP) Diet

The AIP diet was implemented as the foundational clinical tool — removing all foods that can amplify immune reactivity while flooding the body with nutrient-dense, anti-inflammatory foods. For Hashimoto's, removing the intestinal permeability drivers is not a peripheral recommendation: it directly reduces antibody production. Dairy was managed with precision based on the Zoomer findings — low-whey aged cheeses identified as lower-reactivity options rather than blanket prohibition.

Supplement Protocol

  • Natural Desiccated Thyroid (Metavive III) — providing both T4 and T3, more closely mirroring natural thyroid output than synthetic T4 alone
  • Selenium (100µg, twice daily) — foundational for reducing TPO antibody levels. Protects thyroid tissue from the oxidative damage caused by antibody-driven inflammation.
  • Inositol — specific thyroid antibody modulation support
  • Nattokinase — systemic enzyme activity supporting inflammatory protein breakdown
  • QuattroFerrin — iron repletion to address the low ferritin finding (improved from 28.1 to 61–69 µg/L)
  • Stress Nutrients (Seeking Health) — adrenal support addressing the HPA fatigue on DUTCH testing
  • Creatine 10g daily — for fatigue and cognitive function via mitochondrial energy support
  • Evening Primrose Oil for perimenopausal and skin support
  • Hydroxo B12 with Folinic Acid — non-methylated form given possible methylation sensitivities

Nervous System & Lifestyle

A structured nervous system recovery programme grounded in Polyvagal Theory, Somatic Awareness, Vagus Nerve Toning, and Trauma Healing Principles — addressing the deep-seated patterns that perpetuate immune dysregulation, fatigue and anxiety when left unaddressed. Buteyko breathing for mast cell stabilisation, histamine regulation and nitric oxide production. TRE (Trauma & Tension Releasing Exercises) to help the nervous system discharge chronic stored tension. Cognitive biohack using low-dose nicotine patches for focus and working memory.

Why This Worked

The thyroid was addressed at the immune level, not just the hormonal one. Standard management replaces the hormone — but does nothing about the 291 TPO antibodies attacking the gland. Every element of the programme — AIP, selenium, inositol, gut work, dairy management — was chosen because it directly addresses the autoimmune process. The gut was recognised as central to the autoimmune picture: without restoring intestinal integrity and removing the dairy trigger, the antibody reduction achieved would not have been possible. And the nervous system work was not optional — the DUTCH test showed adrenal fatigue at a measurable, biological level that thyroid hormone alone could never resolve. A starting MSQ of 38. A finishing MSQ of 4. Antibodies falling from 291 to 72. These results were the product of a precise, patient, whole-system approach.
